CPT CODE: 83516
NGMC CODE: HU
SPECIMEN REQUIREMENTS: 2 mL serum. Frozen.
DEPT/TURNAROUND: Send Out/2-5 days
METHODOLOGY: IB
REFERENCE RANGES:
Neuronal Nuclear (Hu) Autoabs -IB Negative
Neuronal Nuclear (Ri) Autoabs -IB Negative
Purkinje Cell (Yo) Autoabs -IB Negative
Neuronal Antibody IgG, Immunoblot Amphiphysin Negative

Anti-Hu, type I anti-neuronal nuclear (ANNA-1) is associated with small cell lung cancer. Anti-Ri, type II anti-neuronal nuclear (ANNA-2) is associated with neuroblastoma (children) and fallopian tube or breast cancer (adults). Anti-Yo, anti-purkinje cell cytoplasmic antibodies (PCCA or PCA-1) is associated with ovarian and breast carcinomas.

The presence of one or more of these antibodies supports the clinical diagnosis of paraneoplastic syndrome and leads to a focused search for underlying neoplasm. These markers aid in the discrimination between true paraneoplastic disorders and other inflammatory disorders of the nervous system.

Amphiphysin antibodies are present in about 5% of patients with stiff person syndrome and variably in other paraneoplastic neurological syndrome (PNS) cases. Amphiphysin antibodies are mostly associated with small cell lung cancer (SCLC) and breast tumors.
